No value or non-ident comparison reviews?

I am interested in finding out if any labs are doing full case review of all latents in each case (of value and no value prints) and if any labs are doing 100% verification of all comparisons (ident, non-ident, exclusions and inconclusives). If you are doing any of the above, in approximately what percentage of cases is the second examiner catching errors? Please also provide info on which agency you work for so we are not duplicating data we receive from the same agency. Any information on these issues would be extremely helpful to us!
