Report Wording/Formats

We are in the process of revising our report formats and I am interested to see how other agencies prepare their reports. I work for a state agency, so hearing from other labs similar to ours would be very helpful. If anyone can share a generic report, it would be appreciated. Thanks!

We have already looked at SWGFAST, but I am more interested in how that is being applied by others.
