Case File (Case Record) disclaimer?

Is anyone using this or some form of it in their reports?
This report does not contain all of the information needed to independently evaluate the work
performed or independently interpret the data. Such an evaluation requires a review of the case
record.

We have a disclaimer about if an arrest is made on the above identification, a new card of that arrest should be forwarded to us so we can confirm the identify of the person arrested (but in reality, this NEVER happens).

Just curious about how you all are writing reports.

We have a similar statement. Basically says if we are needed to testify please send us a copy of the defendants record prints so we can compare to what we received from AFIS and release a report saying they are the same. We never get any either. We end up having to go down to the courthouse, take the defendants fingerprints and bring them back and write a new report saying that the records the examiner took at the courthouse match those we received from AFIS.
