I had sent Bill a message about emailing himself the PDF, but I didn't even think of using a dropbox folder. I wasn't sure if the iPad allowed file storage, but dropbox gets around that issue.
It looks like there are a few PDF apps for iPad: Good Reader, PDFExpert, iAnnotate are a few I've found mentioned:

Thanks, Steve. I will give it a shot. I got the iPad recently and haven't fully mastered the use of it. I have to wrestle it away from the rest of the family.

An examiner in our lab just pointed out something interesting. For those that have the printed book version, compare the "enlarged" images of the thumb and index fingers on the cover. Hmmmmm....

If you download this file, you can transfer it straight to your Kindle or Nook via the USB cable or email it to your Kindle email address (I don't have a Nook so I don't know if you get an email address with it). I teach at a midwestern university and I have access to the CRC Press publications website where I can get a ton of their books in pdf format...all the ones that I use or have used I've been able to download and install on my Kindle. It's great having those materials so close.
As far as the enlarged prints on the cover, the right index finger has two deltas on the left side...clearly they superimposed the thumb on the index finger. Here's a picture: I did this quickly so please excuse the sloppiness!
