Questionnaire

Our laboratory is preparing a questionnaire which will be sent out along with the completed report. The questionnaire is really an evaluation of the laboratory services. I have a sample that was used by another department which covers the general questions such as; how was the service? How satisfies were you with the time taken to complete the analysis? Were you satisfied with the reports/statements sent to you?
These are good overall questions but, I was wondering if other departments have a similar questionnaire that may have other questions of value and would be wiling to share those questions. We hope to complete the questionnaire by next week and would be happy to pass along our final draft to anyone who may be interested.

Our AFIS section used to send out a questionnaire with every fingerprint identification. Among the questions were:

(1) Did this information assist in the investigation?

(2) Did it result in an arrest?

We also had a space for comments from the investigators. One of them wrote in, "Fingerprint identifications are very helpful to my investigation."
