Documenting Verification

I'm looking for input from ASCLD-LAB accredited Labs, Legacy or ISO. Could you please explain the process you use to document a verification. For example in our lab we make a statement of verification, by whom and the date completed.
We are considering having the verifier complete a re-examination worksheet containing their own notes for furture reference.
If you could please reply with the program your lab is accrededited and the verification process you use it would be greatly appreciated. You can post a reply or e-mail me if you feel more comfortable. Any and all input is welcome.

My lab is ASCLD-Lab accredited and what I can mostly say is: do what your SOP's say you do. We have in our SOP's that the verifier initials and makes a check-mark and "ID" next to the identified latent print on the lift card/photo. The verifier puts the check-mark and "ID", their initials and the date next to the markings on the exemplars from the original examiner, and finally, they initial and date the worksheet where the ID's were documented by the original examiner. The verifiers do not prepare a separate report.
